Something like this is alright As for appearance, it is alright as long as it is not something major like a sudden change in eye colour or ethnicity (e.g. your character cannot go from brown eyes to blue unless they wear contacts). Something such as a hair cut is fine.

That should be okay! As long as appearance hasn't been described in too much detail in RPs, I dont think changing that sightly would be a problem either

Though I will say, if you are in doubt in regards to [Applications in Index] and Encylopedias you could reach out to an index lead and their second in command. Currently that is Taros Quintero and Meiyu Xiang.

When it came to changing information in encylopedias, when I worked index, I would look to see if there are any conflicting information, if it made sense, whatever. Some people submit minor things as 'major' things for their characters, and those are easy to approve. Others submit major things like possibly a change to the backstory, and those take a bit more efforts and checking to approve. Some people also attach plots that index reads over and approves as they'll affect the character at some point during the duration of it.

The point is, if you are in doubt, you could reach out to an index lead and the second in command. They are in charge of index, and they can discuss within the index team when they aren't sure, to give you a solid answer or some help.

Giving a nickname I don't consider a major thing. It's like Elmo's mom calling him El or Momo. It's just a nickname, not a major change. A major change might be something like adding events to his backstory that weren't there originally, or rewriting it! They'd prefer papertrail to know what was changed and what wasn't so you aren't purposefully removing something that they approved in the first place.
