Do you need to? I really can't answer this, you will have to decide that for yourself.
Do I recommend that you do? Depends on what exactly it includes. How much does this change of backstory include? Are there elements that may turn out being problematic? If yes- then yeah, absolutely. I'm afraid there is not much of a list with criteria when you do or don't have to re-index.

Would it be within the rules to create an invisible door leading to a secret room within the Clock Tower, for a specific RP? No changing the lore or what is already written. Just making an addition to create a new setting for two characters to explore, with focus on their interaction and relationship.

If you're looking for a 'secret room' type location, you can use the Room of Requirement if both of the characters in the thread have fake permission slips! (they can be bought at Weasley's Wizard Wheezes)
