Re-indexing is a means for you to get ensurance that you are fine to write what you want to write. If you believe that your story may become a problem, I suggest to re-index, so the problem can be discovered before it is in the world and causing both you and staff work to fix.
Based on that, I like to let people choose to re-index or not. If they choose not to and the topic becomes a bigger issue, it may leave them with a lot of threads to fix.

Do you need to? I really can't answer this, you will have to decide that for yourself.
Do I recommend that you do? Depends on what exactly it includes. How much does this change of backstory include? Are there elements that may turn out being problematic? If yes- then yeah, absolutely. I'm afraid there is not much of a list with criteria when you do or don't have to re-index.

Would it be within the rules to create an invisible door leading to a secret room within the Clock Tower, for a specific RP? No changing the lore or what is already written. Just making an addition to create a new setting for two characters to explore, with focus on their interaction and relationship.
