Os aren't always perfect scores haha. The only time I've gotten a perfect score on a HIO homework was when I got an extra point for effort and then lost a mark later on, so it wasn't really a perfect score at all xD
edit: wait no, I have, ignore me lol
I havw 5 Os and 1 A - I completely missed out one of the questions by accident whelp. Learned my lesson from that one xD

I usually access HIO on my phone and don't get much data, so I try to remember to do it when I have WiFi and usually end up forgetting whelp
