I'm not sure exactly what bit you wanted in the header and what you wanted the other bit for, so here's three options!

Personally I'd say the first or the third work best as a header, if you like them. But it's up to you! Feel free to use any - or not at all, if you don't like them!

Other Note: I am using this for the signature space on my Student ID Card, so if you are familiar with all that sizing, you may be able to base the sizing off that.
