When it has happened to me it was because my brother was trying to shove me out side and then lock me out and he slammed it on my hand.

Oof, I know that hurt! One time I was very young and mine got stuck under the wheels of a moving shopping cart, and another time slammed and crushed in a car door...then one time my dad closed my leg into the car door even though I SAID I WAS COMING!!!
